The agreement between you and Kura Notes LLC for the use of KODEx. Plain wording, and short where it can be.
In effect from 7 October 2026
KODEx is operated by Kura Notes LLC (“we”, “us”). By creating an account or using the service you agree to these terms. If you are agreeing on behalf of an employer, you confirm you are authorised to bind them, and “you” means both of you.
KODEx is a reference. It gathers what public bodies publish about codes, policies and payment, and shows each statement with its source and the date it was last checked.
It does not decide whether a service is payable, whether a payer covers it, whether codes may be billed together, or what to do about a denial. It is not legal, medical, coding, billing or compliance advice. The limits are set out in full at what KODEx does not do, which forms part of these terms. Read it before relying on anything here.
You are responsible for every claim, appeal and submission you make. Where a payer’s own document and this application disagree, the payer’s document governs.
You agree not to:
We may suspend or close an account that breaches this section, and will say why.
No patient information. KODEx is not designed to receive protected health information (PHI) as HIPAA defines it. Kura Notes LLC is not your business associate and will not sign a business associate agreement. Do not enter patient names, dates of birth or service, member, claim or record numbers, addresses, or anything else that identifies a patient, in any field — including search, notes, drug sheets and problem reports. If you do, you are responsible for that disclosure, and we may delete it.
Most of what KODEx holds is published by the United States government and is not subject to copyright in the United States. Some material, such as commercial payer policy titles and summaries, belongs to its publisher and is shown with attribution and a link to the original.
Some code sets are licensed by the bodies that own them — CPT® by the American Medical Association, CDT® by the American Dental Association, and the CARC and RARC lists by X12 — and their descriptors are not included here. Where a page names such a code, KODEx shows the code itself and, at most, its own plain-language summary. Holding a licence for those code sets, where your work requires one, is your responsibility. Nothing in these terms grants you any right in them.
CPT® is a registered trademark of the American Medical Association, and CDT® of the American Dental Association. KODEx is not affiliated with or endorsed by CMS, FDA, the National Library of Medicine, the AMA, the ADA, X12 or any payer.
We grant you a limited, non-exclusive, non-transferable licence to use KODEx and to print or export reasonable portions for your own and your team’s internal work. You keep ownership of the notes, sheets and other content you enter, and grant us the licence needed to store it and show it to the people you choose. If you send us feedback, we may use it without obligation to you.
We aim to keep KODEx available but do not promise it will be uninterrupted or error-free. We may change, add or remove features, and may change which publishers are covered. Where a change materially reduces what a paid plan does, we will tell subscribers by email before it takes effect.
The service and its contents are provided “as is” and “as available”, without warranty of any kind, express or implied, including any implied warranty of merchantability, fitness for a particular purpose, non-infringement, accuracy or currency. We do not warrant that the data is complete, current or free from error. Some jurisdictions do not allow the exclusion of implied warranties, in which case the exclusions above apply only so far as the law permits.
To the fullest extent the law allows, neither party is liable to the other for indirect, incidental, special, consequential or punitive damages, or for lost profits, lost revenue, lost data, or denied, delayed, reduced or recouped claim payments, however caused and on any theory of liability, even if advised of the possibility.
To the fullest extent the law allows, our total liability arising out of or relating to these terms or the service is limited to the fees you paid us in the twelve months before the event giving rise to the claim, or one hundred United States dollars if you paid us nothing.
Nothing in these terms excludes liability that cannot be excluded by law, including for fraud or for death or personal injury caused by negligence.
You will indemnify and hold us harmless against any claim, loss or expense (including reasonable legal fees) arising from your use of the service, your breach of these terms, or any claim, appeal or submission you made in reliance on it.
You may stop using the service at any time, and delete your account yourself from Settings or by writing tosupport@kuranotes.app. Deleting an account ends its subscription at once, and unused paid time is not refunded. We may suspend or end access for breach of these terms, for non-payment, or if we stop offering the service — in the last case we will give reasonable notice and refund the unused part of any period you have paid for. Sections 5, 8, 9, 10 and 13 survive the end of this agreement.
We may update these terms. For a material change we will give at least 30 days’ notice by email to account holders, and the new version takes effect on the date shown at the top of this page. Continuing to use the service after that date means you accept the change.
These terms are governed by the laws of the State of Georgia, without regard to its conflict of laws rules. The state and federal courts located in the State of Georgia have exclusive jurisdiction, and both parties consent to that venue. Before filing anything, please write to support@kuranotes.app: most things are quicker to fix than to argue about.
If you believe something on KODEx infringes your copyright, send a notice meeting 17 U.S.C. § 512(c)(3) to support@kuranotes.app. We remove infringing material, and close the accounts of repeat infringers.
We aim to meet WCAG 2.1 AA. If something here is hard to use with your assistive technology, write to support@kuranotes.app and we will help, and fix it.
