Where KODEx's data comes from, the notices its publishers ask us to show, and the code sets it does not include.

In effect from 30 October 2026

Every fact in KODEx names the document and version it came from, with a link to the publisher’s own copy. This page gathers the credits and notices those publishers ask for. None of them has endorsed KODEx.

Centers for Medicare & Medicaid Services (CMS)

Most of what KODEx holds is published by CMS, including HCPCS Level II, the Physician Fee Schedule, OPPS addenda, the Clinical Laboratory Fee Schedule, NCCI edits and MUEs, the DMEPOS fee schedule, ASP payment limits, MS-DRGs and risk-adjustment mappings, the Medicare Coverage Database, the Medicare manuals and transmittals, ICD-10-PCS, and ICD-10-CM as CMS and the National Center for Health Statistics publish it. KODEx is not affiliated with, endorsed by or sponsored by CMS or the U.S. Department of Health and Human Services. CMS’s publication of a file is not a statement that KODEx presents it correctly; where this application and CMS’s own document differ, CMS’s document governs.

CPT® (American Medical Association)

CMS’s files carry CPT code numbers, and KODEx shows those numbers. It does not hold or show CPT descriptors: where a page needs a label, it is KODEx’s own wording and says so. CMS publishes the files that carry CPT with this notice, which applies to them:

CPT codes, descriptions and other data only are copyright 2026 American Medical Association. All Rights Reserved. Applicable FARS/HHSARS apply. Fee schedules, relative value units, conversion factors and/or related components are not assigned by the AMA, are not part of CPT, and the AMA is not recommending their use. The AMA does not directly or indirectly practice medicine or dispense medical services. The AMA assumes no liability for data contained or not contained herein.

CPT® is a registered trademark of the American Medical Association. Wherever KODEx shows CPT code numbers it also shows the short notice: “CPT only copyright 2026 American Medical Association. All Rights Reserved.”

U.S. Food and Drug Administration (openFDA)

Data provided by the U.S. Food and Drug Administration (https://open.fda.gov). This includes the NDC directory and drug-label information. FDA has not endorsed KODEx, and provides openFDA data without warranty of any kind.

U.S. National Library of Medicine (DailyMed)

This product uses publicly available data from the U.S. National Library of Medicine (NLM), National Institutes of Health, Department of Health and Human Services; NLM is not responsible for the product and does not endorse or recommend this or any other product.

Package photographs in DailyMed belong to their manufacturers. KODEx never copies them: it links to NLM’s copy, and shows it inside a page only where the deployment has chosen to.

Centers for Disease Control and Prevention (CVX)

Vaccine administered codes (CVX) come from the code set published by the Centers for Disease Control and Prevention’s National Center for Immunization and Respiratory Diseases. Use of CDC data does not imply CDC’s endorsement of KODEx.

Health Care Provider Taxonomy (NUCC)

The Health Care Provider Taxonomy code set is © American Medical Association on behalf of the National Uniform Claim Committee (NUCC); used under NUCC’s permission (licence request in progress). It is read from NUCC’s own published file at nucc.org.

UnitedHealthcare

Titles and summaries of UnitedHealthcare policies are © UnitedHealthcare and are shown with links to the originals on UnitedHealthcare’s site. KODEx is not affiliated with or endorsed by UnitedHealthcare. A commercial payer’s policy is one company’s rule, not Medicare’s, and is labelled that way; read the original before relying on it.

Not included

  • X12: the claim adjustment reason (CARC) and remittance advice remark (RARC) code lists are licensed by X12. Denial codes appear as numbers, with what CMS has written about them.
  • CDT®: the American Dental Association’s dental codes and descriptors.
  • NUBC: the National Uniform Billing Committee’s UB-04 code lists, such as revenue codes.
  • CPT® descriptors: see above; code numbers only.

If you hold a licence for any of these, use the licensed source for the wording. The terms of service and what KODEx does not do say how all of this may be used. Questions about a source: Kura Notes LLC, through the contact address in the terms.
